2022 Benefits


HEALTH INSURANCE (BC/BS of Nebraska)


Eligible the first day of the month following the completion of 60 days of employment and working a regular schedule of 30 hours per week.

Employees have two options to choose from:

· High Deductible Health Plan - HSA Eligible*

· PPO Copay Plan

*HSA eligible plans- employee can open an HSA account at the bank of their choice and take advantage of tax savings at the end of the year when they file their taxes, or choose to have their contributions payroll deducted.
H.S.A contribution Limits: Self-Only up to $3,650 Family $7,300 Catch-up Limit (age 65) +$1,000

DENTAL INSURANCE (AMERITAS)


Eligible the first day of the month following the completion of 60 days of employment and working a regular schedule of 30 hours per week. Hospital pays premium for employee; employee may purchase dependent coverage.

VISION INSURANCE (AMERITAS)


Eligible the first day of the month following the completion of 60 days of employment and working a regular schedule of 30 hours per week. Employee pays premium/s.

FLEXIBLE SPENDING ACCOUNTS


Eligible the first day of the month following the completion of 60 days of employment and working a regular schedule of 30 hours per week.

Provides employees an opportunity to reduce their taxable income through salary reduction which goes to their flexible spending account. Employees can have either or both of the following accounts:

1) Medical expenses (out-of-pocket), up to $2,850/year.
2) Dependent Care up to $5,000* per year. *Certain requirements apply.

403(b) AND ROTH 403(b) TAX SHELTERED ANNUITY


Eligible upon employment. The Hospital has arranged for voluntary 403B (pre-tax) and Roth 403b (post tax) plans, allowing employees to save towards their retirement. Rollovers allowed from several other retirement plans. See HR for more information. Employee Contribution Limit $20,500 Catch-up Contribution Limit $6,500

DISCRETIONARY PROFIT SHARING


Eligible after 1 year of employment and 1,000 hours of work in prior year on either January 1 or July 1, whichever is sooner. The hospital will make contributions to the Employee Retirement Plan on a year to year basis, (based on economic conditions of prior year and Board recommendation). Vesting period is three years.

Employees do NOT have to contribute to the retirement plan to become eligible for profit sharing; it happens automatically once they have met the eligibility requirements.

OTHER INSURANCE/VOLUNTARY BENEFITS - AFLAC


Cancer, Specified Event, Hospital and Accident Insurance. Eligible the first day of the month following the completion of 60 days of employment and working at least 30 hours per week. Employee pays premium/s.

LIFE INSURANCE/ACCIDENTAL DEATH & DISMEMBERMENT


Eligible after 3 months of employment and working a regular schedule of at least 20 hours per week. Hospital pays total premium.

VOLUNTARY/DEPENDENT LIFE/ACCIDENT DEATH & DISMEMBERMENT INSURANCE


Eligible after 3 months of employment and works a regular scheduled of at least 20 hours per week. Employee pays premium/s.

SHORT TERM DISABILITY - Voluntary Benefit for Employees to Purchase


Eligible after 3 months of employment and working a regular schedule of at least 20 hours per week. Employee pays premium/s.

Directions - EMPLOYEE ASSISTANCE PROGRAM


Provided at no additional charge to AMH employees and their immediate family members. The EAP can help find solutions for the everyday challenges of work and home as well as for more serious issues involving emotional and physical well being. Receive 6 sessions per issue, max of 12 per year.

TRAVELCONNECT - TRAVEL ASSISTANCE


Provides a wealth of travel, medical and safety-related services you can access while traveling.

LifeKeys SERVICES


Includes EstateGuidance will preparation, GuidanceResources Online, online identity theft resources and beneficiary services.

PAID TIME OFF (PTO; includes use for vacation, sick, holidays, leave of absence, etc.)


All regular full and part-time employees accrue PTO based on hours worked within each pay period, starting day one of hire. (Worked hours do not include overtime hours.) New hires must complete their 90-day orientation period before using accrued PTO.

HOLIDAY PAY


Eligible from date of hire. Hours worked on New Year's, Memorial Day, 4th of July, Labor Day, Thanksgiving, Christmas are paid at time and one half the employees base rate of pay.

LACTATION ROOM


Available to all nursing employees as needed within one year of the birth of a child and during break periods after one year. Availability is on a first come, first serve basis.

WELLNESS PROGRAM


The wellness center is available to all employees at no cost. Arrangements for use need to be made through the Physical Therapy/Wellness department.
